1. General provisions
These regulations define the rules for the use of dental, consultation, diagnostic, and organisational services provided by ATMOSFERA DENTAL CLINIC sp. z o.o., with its registered office at Aleja Wilanowska 9A / U2, 02-765 Warsaw, KRS 0001189951, NIP 9512628195, REGON 542516480, hereinafter referred to as the Clinic.
These regulations apply to patients, their legal representatives, persons acting on their behalf, and persons contacting the Clinic to arrange an appointment, obtain information, or use the contact form available on the website.
2. Registration and appointment booking
Appointments may be booked by phone, e-mail, through the website contact form, or in person during the Clinic’s opening hours. Submitting the form means expressing a wish to be contacted or to reserve an appointment and may require additional confirmation by the Clinic.
The Clinic may contact the patient to confirm the appointment, clarify the scope of the visit, provide organisational instructions, or inform them about documents needed before consultation or treatment.
3. Cancellation and rescheduling
If the patient cannot attend the appointment, they should cancel or reschedule it by phone or e-mail as soon as possible. For treatment visits, multi-stage visits, or visits requiring longer time reserved for the team, the Clinic may individually determine rules for confirmation, prepayment, or late cancellation.
The Clinic reserves the right to change the appointment date for organisational, medical, or random reasons while offering the patient a new date as close as possible to the original one.
4. Course of the visit and patient obligations
The patient is obliged to provide true and up-to-date information regarding their health status, past illnesses, medications taken, allergies, and other circumstances that may affect the safety of diagnostics and treatment.
The patient should follow the doctor’s recommendations before and after the procedure and contact the Clinic promptly in case of doubt. Information published on the website, including post-treatment recommendations, is supportive in nature and does not replace individual recommendations provided during treatment.
5. Payments and documents
Current prices of services are presented on the website for information purposes or are determined individually after consultation and treatment planning. The final cost of treatment may depend on the scope of services, diagnostic results, and medically necessary changes.
The Clinic issues documents required by law, including documents confirming services performed and settlements, in accordance with applicable accounting, tax, and medical rules.
6. Personal data and electronic contact
Rules regarding personal data processing, information about the contact form, e-mail communication, maps, and cookies are described in the Privacy Policy and Cookie Policy available on the Clinic’s website.
Electronic contact and forms should concern matters related to registration, appointment organisation, or questions about the offer. The patient should not send a broader scope of medical data than is necessary to handle the inquiry.
7. Liability and website availability
The Clinic exercises due care to ensure that information published on the website is current, reliable, and useful. However, the content of the website is informational only and does not constitute medical advice, an offer within the meaning of the Civil Code, or a guarantee of achieving the same treatment result for every patient.

Organisational questions, comments regarding website operation, and appointment-related reports may be submitted by phone at +48 664 242 142 or by e-mail to mickiewiczclinic@gmail.com, unless a different contact channel has been indicated for a particular matter.

9. Final provisions
These regulations are effective from the date of publication on the website. The Clinic may amend them if necessary for legal, organisational, technological reasons or due to changes in the scope of services and registration methods. The current version of the regulations is always available on the website.
