Today, dental implantology is considered the most effective method of restoring missing teeth. It is one of the most advanced technologies with many advantages: a crown placed on a dental implant is both aesthetic and functional, and visually indistinguishable from a natural tooth. Alternative methods of replacing missing teeth include bridges or removable dentures. In most cases, after wearing such restorations, significant reconstruction of bone and soft tissue is required, the cost of which is comparable to or even higher than simple implant placement immediately after tooth extraction.
Dental bridges typically require preparation (grinding) of adjacent teeth (always at least two abutment teeth, as single-supported bridges are not allowed) by 1.5–2 mm around the entire crown circumference, often with prior root canal treatment. Currently, there is no absolute guarantee of long-term success of endodontic treatment, as it is a complex and often unpredictable procedure that may lead within 5–6 years to inflammation and eventual loss of the abutment tooth. As a result, the tooth must be extracted, a new bridge planned, often with more “pontic” teeth and additional abutments acting as “sacrificed” teeth.
From a biomechanical perspective, each tooth should ideally function independently. A tooth in the bone is stabilized by elastic fiber structures that distribute chewing forces evenly. When overloaded (for example, by adding pontics in a bridge), irreversible loss of soft tissue (gum) and bone around the abutment teeth may occur, leading to poor aesthetics, mobility, tooth loss, and inflammation. In such cases, when re-treating the area, there is often insufficient bone and soft tissue, similar to situations where a tooth was previously affected by long-term inflammation and gradual bone loss.
Among removable prostheses supported by natural teeth, we only fabricate telescopic dentures. Only these are capable of properly distributing load onto abutment teeth for many years without destroying themselves or the surrounding bone and gum tissue. This type of prosthesis requires mandatory root canal treatment of all abutment teeth. In our clinic, fabrication of bridges combined with various support elements for removable dentures is not permitted.
After tooth extraction, irreversible loss of bone and gum tissue almost always occurs. Although implants can sometimes be placed even with minimal bone and soft tissue, the long-term prognosis is often unfavorable: lack of aesthetics, inflammation, peri-implantitis, implant loss, and the need for more complex re-implantation.
Implant placement involves inserting a titanium implant into a precisely prepared bone site, forming the soft tissue profile, and attaching a prosthetic crown.
**Implantation methods:**
If conditions allow, implants can be placed immediately after tooth extraction (immediate implant placement) or after the healing of the extraction socket (delayed implant placement). The optimal timing is 8–12 weeks after extraction. Bone and soft tissue augmentation can be performed in both approaches.
**Implant procedure:**
Before implant surgery, a surgical guide is recommended. It allows highly precise preparation of the implant site exactly according to the preoperative CT scan data.
